Increase your chances of conceiving the gender you want through sperm sorting

It’s true. You can increase your chances of having a boy or a girl using MicroSort®—the only scientifically proven way to influence gender prior to conception.

MicroSort is a scientific method used before conception to separate sperm into two categories:

    • Sperm that produces girls (X-bearing)
    • Sperm that produces boys (Y-bearing)

Why does sperm sorting help gender selection?
Without assistance, most sperm samples are close to 50% X-bearing and 50% Y-bearing. On your own, your chances of having a boy are about equal to your chances of having a girl.

MicroSort can help you increase those odds—whichever way you choose—by producing a high percentage of either X-bearing (girl) or Y-bearing (boy) sperm.

How does MicroSort separate X- and Y-bearing sperm?
The process relies on a difference in the amount of genetic material in X- and Y-bearing sperm. X chromosomes have slightly more DNA than Y chromosomes. Therefore, X-bearing sperm are slightly larger than Y-bearing sperm. MicroSort distinguishes this size difference using a technique called “flow cytometry.”

Here’s how flow cytometry works:

    1. Your sperm sample is dosed with a safe fluorescent dye, which temporarily attaches to its DNA. Because X-bearing sperm have slightly more DNA, more of the dye will bind to them than to Y-bearing sperm.
    2. The dyed sperm are sent through the flow cytometer, where they travel single-file through a "sheath" fluid.
    3. The sperm then pass through a laser, which makes the fluorescent dye glow. Brighter-glowing cells are sorted as X-bearing sperm.
    4. The resulting enriched sample is not 100% X or Y, but has a significantly higher percentage of one or the other.

What happens after the sperm is sorted?
For most couples, the sorted sperm are used with simple intrauterine insemination (IUI) to achieve pregnancy. Couples needing additional assistance can use MicroSort with in vitro fertilization (IVF). Process details vary depending on which method you use.

For IUI with MicroSort, simple cycle monitoring tests are performed to determine the time of ovulation. Insemination is performed very close to that time to maximize the opportunity for a pregnancy.

To achieve IUI pregnancy with MicroSort, on the day of ovulation the following steps are performed:

    1. Your husband produces a sperm sample.
    2. The sample is sorted for the desired gender.
    3. Insemination with the sorted specimen occurs later that same day.

To achieve IVF/ICSI pregnancy with MicroSort, on the day of ovulation the following steps are performed:

    1. Your husband produces a sperm sample.
    2. The sample is sorted for the desired gender.
    3. The sorted sample is sent to an embryology laboratory for egg fertilization.

Does MicroSort provide IUI or IVF procedures?

To complete the process, patients should select a physician to perform the IUI or IVF procedure from our list of participating physicians. In some cases, a specimen may be sorted at the MicroSort laboratory for use by your local physician.

MicroSort is not for sale in the United States. In the United States, MicroSort is available only to qualified participants through a clinical trial.

MicroSort results are determined by measuring the percentage of sperm for the desired gender in a sample. We can’t guarantee a child of the preferred gender, since our technology doesn’t completely exclude either female or male sperm cells from the enriched sample. However, success rates for producing the desired gender are very high.

How many tries does it take to get pregnant?
Pregnancy rates depend on many factors, including:

    • Age of the mother
    • Quality of the sperm sample
    • Type of clinical procedure (IUI or IVF)
    • Other fertility factors

The pregnancy rate for MicroSort plus IUI is approximately 15-20% per attempt. The pregnancy rate for MicroSort with IVF/ICSI is approximately 35-40%.

Many couples become pregnant on the first attempt. But on average, it takes three to four attempts to become pregnant. MicroSort plus IUI is comparable to natural conception for fertile couples, which generally takes an average of two to three months.

As of January 2008, over 1,300 pregnancies have been achieved using MicroSort. Of those, over 1,200 babies have been born. Many more are still on the way!

How does MicroSort define family balancing?

    • You want to sort for an over-represented gender in your family

To prove you qualify to use MicroSort for family balancing, you must provide copies of birth certificates for all your children. Any circumstance inconsistent with our standards for family balancing or that avoids full reporting of all children may prevent your participation in MicroSort.

If your pregnancy will involve a gestational carrier, MicroSort will only provide sorting for one pregnancy attempt at a time. This policy helps avoid a family imbalance due to multiple simultaneous pregnancies.

Additional vials of sorted semen may be released only if:

How does MicroSort help reduce the risk for X-linked disorders?
Most X-linked disorders affect males more than females. These include several forms of X-linked mental retardation, Duchenne muscular dystrophy and other X-linked muscular dystrophies, as well as hemophilia and many other less common X-linked diseases.

If you’re at risk for transmitting an X-linked genetic disorder, reduce your risk of having an affected child by using MicroSort to increase your chances of having a girl. Will freezing damage sperm?
Frozen sperm has been used successfully for insemination and pregnancy attainment for many years. Cryoprotectants are normally added to the semen sample to protect the cells during the freezing process. However, freezing and thawing does reduce the number of viable sperm cells available for insemination. gender selection

The combination of cryopreservation and MicroSort sperm separation is currently best utilized with IVF and ICSI procedures. The feasibility of using frozen sorted sperm for IUI is currently unknown.

What precautions are taken to prevent switching our sperm sample with someone else's?
We take every precaution to keep your sample safe. When received, your sample is assigned and labeled with a unique identification number. This ID number is maintained throughout the process to assure accurate identification and tracking of each specimen.

Is it possible that my husband naturally has an unequal proportion of X- and Y-bearing sperm cells?
We have performed more than 5,000 analyses of X- and Y-bearing sperm cells prior to sorting and all results have been very close to 50:50. This seems true even when there’s a family history of mostly female or male offspring.

Do you have a waiting list?
Due to limited capacity, there may be a waiting list depending on when you desire to initiate a MicroSort cycle. However, we do our best to minimize delays. We recommend scheduling an appointment several months in advance of the time you want to use MicroSort. If a particular month's schedule is full, we’ll schedule you for the next available opening.

Is MicroSort ethical?

100% Sex Selection Success

The selection of gender has been a quest of couples for as far back as recorded history allows. Early drawings from prehistoric times suggest that sex selection efforts were being investigated by our earliest ancestors. Later history shows intense interest in sex selection by early Asian (Chinese), Egyptian and Greek cultures. This is followed by documented scientific efforts beginning in the 1600's to sway the chances of achieving a pregnancy by a variety of methods.

Research and work carried out in the 1980's and 90's have finally provided methods offering the chance of obtaining a desired pregnancy gender outcome that ranges from excellent to virtually GUARANTEED.
Scientific Understanding

It has been known for many years that the gender of a pregnancy is determined by the sex chromosome carried by the sperm. Sperm bearing an "X" chromosome, when united with the "X" from the female (females only produce "X") will result in an "XX" pregnancy that produces a female. If a sperm bearing a "Y" chromosome (men have both "X" and "Y" bearing sperm) unites with the "X" chromosome from the female, an "XY" pregnancy will result that gives rise to a male offspring. gender selection

Armed with this knowledge, science initially worked to allow for an accurate method of safely separating sperm to allow the majority of those sperm capable of producing the desired gender ("X" sperm or "Y" sperm) to be exposed to the female egg (oocyte). While a variety of methods of purifying the sperm separation process have been reported and studied, in reality, very few of these methods have withstood scientific scrutiny that "checks" the validity of claims made by those employing the procedure.

Because no sperm separation method thus far developed has produced the high level of sperm separation "X" (for female) and "Y" (for male) needed to provide gender outcome success levels greater than 90%, further work to perfect the sex selection process is being studied.

"PGD" (preimplantation genetic diagnosis) has taken sex selection to the next and most successful level ever (greater than 99.9%). Results from our PGD process far exceed reported results from any and all other processes.

Sperm that have been filtered by our standard sperm preparation process are allowed to fertilize the eggs obtained from the female "in vitro" (in our highly specialized fertility laboratory). The embryos resulting from this specialized fertilization process are then screened by our genetics team to determine both their gender and that selected chromosome pairs have resulted in an expected normal genetic pairing outcome (this process is called "aneuploidy" screening). This gender determination process at the very early development level as made famous by our Center, has resulted in the ability to provide sex selection results for the chosen gender far in excess of 99.9%. gender selection

The aneuploidy (abnormal chromosome count) screening process also employed at the time of PGD gender determination also allows for the detection of limited genetic count abnormalities as a routine or for the optional screening of the embryos for a wide variety of additional genetic abnormalities. Upon request, we can screen for genetic abnormalities such as Down's syndrome (one "extra" chromosome 21), Turner's syndrome (the absence of one of the two "X" chromosomes normally found in a female), and Kleinfelter's syndrome (a male with one "Y" chromosome and 2 "X" chromosomes instead of the normally found single "X" chromosome).

New DNA microarray technology also provides us the option of screening embryos for a full (46 chromosome) genetic count. We are also able to provide those patients known to carry specific personal or family genetic diseases the ability to screen the embryos for many specific disorders. All couples meeting our standard, liberal entrance criteria will qualify for the PGD process.

Aneuploidy screening as described above detects abnormal chromosome numbers and the diseases associated with those conditions. "Single gene disorders" include a wide variety of hereditary diseases found on a specific chromosome that can also be screened for with PGD. Our PGD results have been checked and double checked employing the services of a Board Certified geneticist with PGD accuracy better than 99.9%. Because "nothing is perfect" we advise that, with a 99.9% quote of success for desired gender outcome, and 92-99% chance of genetic disease detection, no offer of a 100% guarantee is made or implied.

Unlike many programs offering sex selection only to very limited couples with known genetic disorders in the family we make sex selection available to all patients. Parents have come to us from from nearly every nation on the planet (we have assisted patients from 147 different nations) seeking to balance their families or assure themselves that a pregnancy will result in ONLY the gender outcome they desire. REMEMBER: Nearly all couples qualify for sex selection using the PGD method, which provides near 100% (99.99%) success regardless of sperm counts or gender percentages!

Imagine the following scenarios, which are taken from our clinical files: gender selection

Couple 1: Mr. & Mrs. Adams (names are fictitious) present to our offices with a request for gender selection. The couple reports having successfully conceived and delivered three healthy females. The last birth was three years ago. Mr. Adams has a strong family history of girls being born, with his only brother having produced two girls, and three cousins also having had a total of seven female and one male offspring.

Mr. Adams underwent genetic analysis of the "sex ratio" (percentage of "X" and "Y" sperm) of his total sperm production. His ejaculate was found to contain 51% of the viable sperm seen carrying a "Y" (male producing) chromosome. His total sperm count was 38 million per ml. So, of the 38 million sperm, 51% had the correct sex chromosome needed to produce a male. After separating his sperm for the desired gender, we would be left with a sperm count of at best, 15-16 million. This would not be enough to allow for a reasonable chance of the couple conceiving with simple sperm selection and insemination.

Advised of the situation concerning their sperm sex ratio, the couple wisely elected to proceed with the IVF-PGD option. They were rewarded with a twin pregnancy that resulted in the birth of two healthy, male infants. While we exist to provide high quality medical services, we very strictly adhere to guidelines that have a history of providing excellent outcome results. While we cannot "guarantee" a desired outcome to anyone, we can now come as close to a guarantee as science allows. With the IVF-PGD option, success rates approach 100%. Couples in our program can feel comfortable that once treatment begins, they do have a nearly 100% excellent chance of achieving the desired outcome when pregnancy occurs. Consider the next scenario:

Couple 2: This British couple presented to us with a history of having produced three boys over the past seven years. With very few female offspring in either of their families, they were interested in sex selection aimed at the production of a female.

They undertook an initial telephone consultation with Doctor Steinberg that indicated their suitability for our program so long as some initial blood tests the Doctor requested were in order. The blood testing was ordered from a hospital laboratory near their home in London. One week later, the initial blood screening results showed them to be excellent candidates for the sex selection procedure. Total sperm count on the male was 88 million per ml. Sex ratio demonstrated only 38% of his sperm to be "X" (female) producing.

The couple were directed to one of the several fertility centers in London that we work with who assisted us in the preparation of the patients for the procedure. They began their procedure under the co-direction of our program working with the medical team in London. Two weeks later, they arrived in Los Angeles. Their travel and lodging arrangements had been coordinated with our travel desk who had a car meet them at the airport in Los Angeles. They were transported to the hotel they had chosen and the next morning undertook the egg harvest at our facility.

After our modified sperm preparation separation, we obtained 34 million sex selected sperm per ml. from the husband. The PGD analysis of the embryos produced after insemination of the eggs demonstrated 3 healthy female embryos and 7 healthy male embryos, along with 2 abnormal embryos. Two normal female embryos were transferred to the uterus. The couple returned home after two "extra" days of tourism in Los Angeles and two weeks later a pregnancy test was positive. A single healthy female birth resulted.

Couple 3: Lisa and David were referred to us by a local fertility program near their home in Toronto, Canada. They presented to their local program with a request to assist them in their desire to become pregnant with a boy.

Lisa underwent a tubal ligation 3 years earlier after the birth of their third daughter. Lisa and David explained that they felt that they had "reached their limit" after having 3 children and undertook the tubal ligation. They indicated that with their oldest daughters now growing older and able to help with the youngest one, their thoughts had changed and they now felt that they could care for an additional child.

While totally content with their daughters, David indicated that he was very interested in seeing if the couple could use "new science" to help them achieve the birth of a son. Lisa was very supportive and indicated that she too was ready for another child. She indicated that gender was not a concern for her but that she was ready to support David's decision to inquire about our gender selection program.

They were told by their local fertility physician that gender selection was illegal in Canada. He did advise them that he had seen and assisted in the care of several Canadian patients that were being treated for sex selection at our Center in Los Angeles. He suggested to Lisa and David that they come see us. The couple had an initial telephone based consultation with us. This was followed by the performance of blood tests and a semen analysis that was carried out by a laboratory near their home.

We performed a sex ratio on David's sperm that provides us with very valuable information about David's capacity to produce male embryos. After we learned that the couple seemed to be suitable candidates for the procedure, they underwent their initial start-up examinations at the local center. We are able to interface with physicians near the homes of patients in nearly all cases.

Because Lisa was going to be 39 years old at the time of her delivery, the couple opted to also check their embryos genetically to make sure that a pregnancy with Down's syndrome did not result.

Lisa and David achieved a successful male pregnancy that resulted in a healthy baby boy born at Toronto General Hospital. Their referring physician has sent us several additional patients and two of David's friends have now been seen with a request for gender selection.

Sex Selection FAQs

Sex selection is a very difficult procedure to obtain in the United States and world wide. Our Center has the world's largest experience with PGD sex selection, and we are reporting a 100% gender outcome success rate with all patients seen at our facility. Our unmatched, extensive experience with gender selection and our review of world literature allows us to offer the following information and answers to questions:

How can I assure that I am choosing the BEST center to carry out my procedure?

This is perhaps THE most important question that can be asked. Of the few centers that offer this highly advanced procedure, there are only a very few that provide PGD SUB-SPECIALIST Ph.D. EMBRYOLOGISTS to carry out the critical biopsy on your newly developed embryos.

We have recently heard from several patients shopping sex selection programs that very few programs were able to guarantee that a Ph.D. SUB-SPECIALIST EMBRYOLOGIST (a DOCTOR-SCIENTIST DEDICATED TO EMBRYO BIOPSY FOR SEX SELECTION) would be available to perform this highly complex procedure. They were advised that IVF technicians were "adequate" for the procedure. We disagree. We suggest that you request IN WRITING an assurance that a Ph.D. SPECIALIST in embryo biopsy will carry out your procedure.

Request the NAME of the DOCTOR and request that ONLY this Doctor perform the procedure. As in our Center, other Centers dedicated to the highest quality in sex selection science will have no problem providing you the name of the Doctor performing this important laboratory service. Accept NO excuses or alternative explanations for why such a specialist will not be performing your biopsy.

Ask if there is a full time, on-site Ph.D. embryologist at the facility. While the recent cost-saving trend has been to allow the highly qualified laboratory directors to be "traveling consultants" (that is, they work at outside programs or Universities and staff the local facility with lesser qualified technicians, only visiting the Center intermittently), at The Fertility Institutes, we feel this to be a less than optimal arrangement that has the potential to compromise your chances for success.

We maintain a FULL TIME-IN HOUSE Ph.D. director. If you read that the "Laboratory Director" at a facility is a "consultant" or other synonym, and is NOT at the Center FULL TIME, you are well advised to reconsider this program, even if the costs, on the surface, appear to be lower. The fact is, highly skilled specialists, as with most things in life, are a more costly initial option.SERVICE FOR ADDITIONAL INFORMATION In the end, however, if your desired result is not obtained, the small savings obtained by using inferior methods is no savings at all. Has sex selection produced a reasonable number of successes?

YES! While success rates vary between programs, most programs employing standard sperm separation procedures report success rates in the 60-68 percent range. Success with IVF-PGD approaches 100% worldwide. Our program reports an 65-70% chance of achieving the desired gender outcome with sperm separation and a 99-100% assurance with IVF-PGD.

Are fertility drugs used in sex selection procedures?

Yes and No. Sex selection being carried out on normally fertile couples who have already produced one or more offspring and are using the sperm separation procedure do not normally require fertility drugs gender selection. Our sperm separation/sex selection procedures are offered, however, to couples needing and undertaking fertility treatment as well. In these cases, the drugs normally used for these fertility procedures are combined with the sex selection procedure. In the case of IVF-PGD, and MicroSort with IVF, fertility drugs are used consistent with our normal IVF procedures.

How many babies have been born as a result of sex selection?

Thousands of healthy babies have been born following sex selection procedures. In the case of the MicroSort method, MicroSort is a clinical trial to prove safety and efficacy. This procedure uses an investigational device, which is limited by federal law to investigational use. MicroSort has over 400 babies born with the MicroSort technology. The PGD method has been used to evaluate the genetic health of embryos for several years and the technology, while being developed carefully, has been proven effective.

Is there an increased risk of a birth defect after the sex selection procedure?

No. Many studies have been carried out on artificial insemination procedures. These studies, looking at fresh, frozen, washed and isolated sperm show no increase in the occurrence of birth defects when compared to natural conception. In the case of IVF-PGD, while relatively new, there has thus far been noted no increased risk to the offspring.

It should be remembered that PGD is nearly 100% effective (99.9%) and is being studied closely to see how costs for the procedure can be reduced. For this reason, we now recommend PGD as the primary treatment option to couples requiring gender selection for where more than 1 child has been born of the gender opposite the gender now being sought by the couple. How is the sex selection procedure carried out?

Preparation for your procedure will occur well in advance of your treatment start date. Actual preparations can begin as early as just after your consultation with the Doctor and acceptance into the program. Our scheduling staff will work closely with you to assure that your procedure will be scheduled for a date most convenient for you.

Due to the large demand for our services, it is most important that you begin the scheduling process as soon as possible after deciding to undertake the PGD procedure. Once the specific details of your needs are obtained (local monitoring ad preparation with us in Los Angeles, preparation near home for those at a distance, etc.) our staff will arrange for the necessary pre-procedure laboratory testing.

If out of town, you will be provided with information on a monitoring center near you that will assist with the preparations. Our nurses can arrange and facilitate your appointments with these centers. Due to the special arrangements we have with many centers, we can nearly always assure that you will be provided appointments rapidly, even in centers with long wait lists for standard services.

We will arrange for your medications to be ordered and you will be given all preliminary instructions. For local patients, you will be scheduled for visits with us in accordance with he Doctor's orders and schedule plan for your treatment. You will be provided details about calling us with the onset of the menstrual period that occurs during the month your treatment is scheduled for. Once this occurs, we will assist with scheduling all of you planned monitoring visits.

Following retrieval of the eggs with IVF, the eggs are inseminated in the laboratory with sperm that has been filtered and processed for use with the PGD procedure. These sperm are then used to fertilize the healthy eggs by the embryologist.

After a few days growing in the incubator, the resulting embryos are gently biopsied to allow us to obtain a cell that may be used to determine the sex (gender) of each embryo. Once this information is available (usually within 24 hours), we can select only those embryos of the desired gender for implantation.

Options are offered for the disposition of the remaining embryos that include the ability to cryopreserve the embryos or offer the embryos for embryo adoption to other couples in need of embryos.

Sex selection

The term ‘sex selection’ is used to refer to various processes that allow you to choose the sex of an embryo.

You might want to do this for medical reasons, for example, to avoid giving birth to children with a sex-linked genetic disorder like Duchenne muscular dystrophy.

These diseases affect boys but not girls (girls may still ‘carry’ the gene for the disease but they will not suffer from it).

You may want to select the sex of an embryo for personal reasons such as wanting to balance your family. In the UK sex selection is only allowed for medical reasons.

Is sex selection for me?

The only situation in which you should be advised to use sex selection is in the case of medical need, such as being at risk of passing on a known genetic disease that affects children of one sex only. In this case, it is acceptable to select the sex of an embryo so it will be unaffected by the disease.

How does sex selection work?

In the UK, Pre-implantation genetic diagnosis (PGD) is currently the only method used in practice to choose the sex of the baby.

    * Pre-implantation genetic diagnosis (PGD)

You may hear about the other methods listed:

Sperm sorting – this is where sperm are selected according to whether they carry male or female chromosomes.

A sample of the chosen sperm is then used to inseminate a woman or create IVF embryos in the lab. The only method of sperm sorting that is currently permitted in the UK is flow cytometry, which uses fluorescent dye to separate sperm carrying male chromosomes from those carrying female ones.

This method is not 100% reliable, so it is not used in practice.

Folklore and ‘natural’ methods such as the timing of intercourse to favour the conception of a child of a particular sex.

Selective abortion of foetuses that are shown by ultrasound to be the sex that is not wanted.

Since the changes to the EU law in July 2007, all clinics that offer sperm processing must be licensed by the HFEA and must only offer sex selection for medical reasons.

What are the risks of sex selection?

Most of the risks involved in sex selection treatment are similar to those for conventional in vitro fertilisation (IVF). For more information, see:

With sex selection, there is also the possibility that:

    * some embryos may be damaged by the process of testing
    * no embryos are suitable for transfer to the womb after sex selection (i.e. all embryos are of the sex being selected against)
    * flow cytometry is not 100% reliable, which is why it is not normally offered.

They do so mainly for two reasons. First, couples who have children of the same gender want to experience the joy of raising a child of the opposite sex. Second, couples want to shield their future children from inheriting a genetic disease.
Baby Gender Selection Methods

The Silverman Center for Gender Selection offers two techniques to choose baby's gender, the Ericsson Method and the IVF/PGD Method combined with the Ericsson Method of sperm separation.
The Ericsson Method: More Cost Efficient

With this technique sperm are sorted by gender, and the sperm of the sex that the couple desires are used for an intrauterine insemination. The major advantage of this method is that it is less expensive than the IVF/PGD Technique. Numerous scientific studies document that the Ericsson Method of Gender Selection is effective 70-75% of the time.
The IVF/PGD Technique: Highest Success Rate

Through a process called IVF/PGD, combined with the Ericsson method, Dr. Silverman enables eggs to be fertilized with the father's selected sperm outside of the mother's body.

The embryos are tested for their gender and only the embryo of the desired sex is transferred back to the mother.

Parents frequently prefer their offspring to be of a particular sex for a variety of reasons. Social sex selection or human sex selection is the attempt to control the sex of the offspring to achieve a desired sex. It can be accomplished in several ways, both pre- and post-implantation of an embryo, as well as at birth. It has been marketed under the title family balancing.

In some cultures, male offspring are desired in order to inherit property, carry on family name, to provide support for parents in old age.

Studies have demonstrated that having sisters, as compared to brothers, can enhance the quality of life of an adult.[1]

Evidence suggests couples in the United Kingdom tend to pick sons and daughters in roughly even numbers.[2]

Most clinics in the United States treat only those parents who already have a child of the other sex. [2]

Two major types of pre-implantation methods can be used for social sex selection.

Sperm sorting - The separation of X Chromosome sperm from Y Chromosome sperm. The resulting sorted sperm are used in either artificial insemination or in-vitro fertilization (IVF) procedures.

Preimplantation genetic diagnosis (PGD) - In sex selection cases, embryos resulting from IVF procedures are genetically tested for X or Y Chromosomes. The embryos of the desired sex are then implanted.

No "seamless" method is known to exist where couples take certain measure before sexual intercourse normally. Even if there were one, many countries would refuse to grant patents to implements as contravention of common decency.
[edit] Post-implantation methods

Prenatal diagnosis - Amniocentesis and/or ultrasound is used to determine sex of an offspring, leading to subsequent abortion of any offspring of the unwanted sex. The more recent technique of fetal blood now makes it possible to test the sex of the fetus from the sixth week of pregnancy.

Sex-selective infanticide - Killing children of the unwanted sex. Though illegal in most parts of the world, it is still practiced.

Sex-selective child abandonment - Abandoning children of the unwanted sex. Though illegal in most parts of the world, it is still practiced.

Sex-selective adoption - Placing children of the unwanted sex up for adoption. Less commonly viewed as a method of social sex selection, adoption affords families that have a gender preference a legal means of choosing offspring of a particular sex.
[edit] Ethical concerns

The application of these techniques to humans creates moral and ethical concerns in the opinion of some, while the advantages of sensible use of selected technologies is favored by others.

Listed here are some ethical concerns:

Sexual discrimination - The idea that if one sex is preferred ove another, those individuals in the non-preferred sex would be at a disadvantage. Opponents of social sex selection argue that the procedure would artificially unequalize the ratio of females to males, leading to discrimination, potential violence and abuse of the smaller group.

Eugenics - Many fear that PGD, in general, is a 'slippery slope' leading to a society where 'non-selected' individuals would be discriminated against. PGD is used primarily in the U.S. for the purpose of reducing birth defects and abnormalities, but opponents fear that there is nothing stopping persons from using PGD for more eugenic-based purposes. It has been noticed that the largest gender imbalances in China come from the Han Chinese ethnic group.[1]

Psychological Implications - There may be psychological implications for both the parents and child if the procedure does not produce a child of the desired sex. Furthermore, problems may also arise if the gender-related expectations of the parents are not subsequently fulfilled by the child. However, it may be the case that any child will fail to fulfill particular parental expectations, so perhaps more emphasis should be placed on promoting acceptance and tolerance within parents as opposed to completely banning sex selection.

Conflict with Kantian principles - Many argue that by selecting the sex of their child, parents are using the child as a means of fulfilling their own desires rather than respecting the child as a person and an end in their own right.

In contrast, there is widespread support for the concept that individual reproductive choice is an important private decision which should not be infringed by government. There is considerable evidence from sperm sorting in the United States that pre-conceptual use of this technology, which does not involve destruction of embryos or fetuses, is desired and utilized by many couples to achieve balancing of gender ratios within their families. Furthermore, the fear that there would be preferential selection of boys is clearly false - indeed, actual experience in the U.S. indicates that the technology is used more often to obtain girls than boys. Thus the real-world experiences with techniques such as MicroSort demonstrate that theoretical fears of gender discrimination from sperm sorting are not at all realized in a country such as the U.S. There is equally little real evidence in support of the other theoretical harms postulated by opponents of pre-conceptual gender selection.

Post-conceptual selection by preimplantation testing (PGD) is a distinct subject as it obviously involves preferential use of embryos, and of course termination of pregnancy for gender selection is a quite different matter.
[edit] Demographic concerns

In addition to the ethical concerns mentioned, issues of demographics arise in societies where social sex selection is common. A society may exhibit a widespread bias towards having children of a specific gender, either due to cultural biases or economic concerns (e.g. male children may be more employable in the future and thus provide more financial support). When combined with frequent social sex selection, this bias may produce a gender imbalance that has undesirable consequences. This phenomenon has been observed in many nations in the Far East, such as India and China, where social sex selection has produced unnaturally high male/female ratios in the population. China's gender imbalance is further increased by the One Child Policy, although applicable only in most urban populations. In these nations, a lack of opportunity for many men to marry is believed to be producing increases in crime, demand for prostitution, and the selling of brides.[3][4]

Major demographic changes have occurred in China because of sex selection. Chinese government reports show that the sex ratio for newborns is 118:100 (boys:girls), higher in rural areas such as Guangdong and Hainan (130:100) compared to the average of 104:107 in developed countries. It is believed that the ratio would increase further to the point that, by 2020, men of marriageable age would be unable to find mates, resulting in large social problems.[2][3]

Some nations, such as India, have attempted to curtail these gender imbalances with criminal statutes.[5] In contrast, bioethicist Jacob Appel of New York University has argued that governments should pay couples to choose to have female children.[6] Acccording to Professor Appel, "if female babies [were] worth their weight in rupees and yuan, economic and educational opportunities for girls would soon follow."[7]

In contrast, actual experience in Western cultures provides no evidence for any degree of gender imbalance from technologies which have long been available and legal - such as selective abortion or preimplantation embryo testing. When used for family balancing indications in such countries as the United States, pre-conceptual sex selection is widely sought without any preferential selection of males. Thus the right of individual families to determine whether or not to balance gender of offspring in their families is not and will not become, in many countries, a demographic issue. Furthermore, in countries where such demographic issues exist because of strong gender preferences in a segment of the population, regulatory and legal control of, without denial of access to, sperm sorting technology can be utilized to provide individuals with choices while ensuring that equal numbers of boys and girls are produced for population demographic equality.
[edit] History and folk beliefs

There are a wide variety of social sex selections methods which have not been demonstrated to be effective. Because even implausible and ineffective methods have a "success" rate of 50%, many continued to be recommended by word of mouth.

As early as 330 BC, Aristotle prescribed the ligation (tying off) of the left testicle in men wishing to have boys.[8]

Some people believe that timing conception according to astrological charts can influence a baby's sex,[9] though there is no evidence to support this or any other timing method. A 13th century Chinese conception chart purports to be able to identify the sex of the baby before birth.

During the 1980s, Lawrence Livermore National Laboratories teamed with the United States Department of Agriculture to develop the first (and only) successful and repeatable means of separating dead X- (female) and Y- (male) chromosome bearing spermatozoa.[10]. It was not until the 1990s that the United States Department of Agriculture and others modified the discovery for commercialization.It was not until the 1990s that the United States Department of Agriculture modified the discovery for commercialization. 'Sexed semen' is offered commercially in cattle by a variety of companies including Cogent (UK) and Sexing Technologies (USA). In humans the technique is marketed as 'MicroSort', but is unavailable outside of the USA due to legal restrictions.

Recently, a study published in 2006 indicated that mothers with toxoplasmosis have a significantly higher sex ratio of boys to girls. This has been discussed in connection with the manipulation hypothesis of parasites.[11] Another study found a link between sex and the diet of the mother, but this may be due to statistical chance, and has yet to be confirmed.[12]

Sex selection is officially prohibited in China, but the Chinese government admits that the practice is widespread, especially in rural areas of China and among lawless groups such as ghettoized migrant workers in cities (despite denials by the government-sponsored studies) [4][5]

Social sex selection is illegal in India. To ensure this, prenatal determination of sex through ultrasound is also illegal in India. These laws are instituted to combat the prevalent practice of sex-selective abortion. However, these laws have generally failed to be effective in rural areas and, despite education efforts, sex-selective abortion continues to be widely practised there.

Sex selection is legal in most of the world, and it's practiced particularly in Western countries, but is more limited in Eastern countries, such as India or China. However, an exception of this is Europe, where the practice of sex-selection is prohibited (bar the Turkish Republic of Northern Cyprus). There is fertility tourism from the United Kingdom to the United States for sex selection, because preimplantation genetic diagnosis (PGD, a potential expansion of IVF), which can be used for sex selection, is prohibited in the UK, except when it is used to screen for genetic diseases, while the laws in the US are more relaxed in this subject.[2]

MicroSort®:
The MicroSort® process sorts sperm by male and female by a process that measures differences in the DNA. Then using an intrauterine insemination (IUI) or in vitro fertilization (IVF) the enriched sample of sperm is used to help you conceive. About 92% of those attempting for a girl do conceive a girl, while the success rates for sex selection and boys using MicroSort® is lower at 81%. The pregnancy rate for using an IUI cycle is 15.6% and the overall IVF/ICSI clinical pregnancy rate is 32%.

Preimplantation Genetic Diagnosis (PGD):
This is the most effective technique in sex selection. With nearly 100% accuracy and the ability to select not only sex but a healthy embryo makes it very popular. Though this is a costly and intensive method of sex selection that uses in vitro fertilization and does biopsies on the embryos before they are returned to the uterus.

    * Preimplantation Genetic Haplotyping (PGH)

Shettles Method:
The Shettles Method is based on the fact that male and female sperm travel and survive in the reproductive tract for varying amounts of time. So you time intercourse about 12 hours prior to ovulation for a boy and several days before ovulation for a girl. This is a very simplified explanation of Shettles.

Ericsson Albumin Method:
With Ericsson, the sperm is filtered through albumin and then an intrauterine insemination (IUI) is done with the sample. While the sample doesn't provide you with more of one particular sex, like a MicroSort® sample, it does help select the sex of the baby. For a girl, Clomid® is used since it has been shown to increase the number of girls.

Other Methods of Sex Selection:
There are other methods of sex selection, including prepregnancy diets to alter the pH balance of your body, timing methods that differ from Shettles, O+12, supplements, and positioning. These low tech methods are not as accurate as MicroSort® and PGD and are sometimes dangerous.

Finding Out if Sex Selection Worked:
There are four basic ways to find out if sex selection worked:

    * Preimplantation Genetic Diagnosis
    * Prenatal Genetic Testing (Amniocentesis or Chorionic Villus Sampling (CVS))

Each of these has their own risks and benefits. Some are even more accurate than others. They also vary as to at what point in or before pregnancy they can be done.
